Application for TLC Cat Spay/Neuter Vouchers

Program

Our spay/neuter program provides free cat spay/neuter vouchers to low-income households within our service area. They cover the full cost to spay or neuter your cat and one vaccination each for rabies and distemper provided they're done at the time of surgery. The vouchers are good for 30 days and are usable only at the veterinary clinic listed on them. There is a lifetime limit of 3 vouchers per household.

Eligibility

Submit an application only if you meet all of the following requirements:

You live in one of the zipcodes listed below.
Your annual household income is under $40,000.
You are at least 18 years old and the head of household.
You are the primary caregiver for the cats applied for, have been so for at least 30 days and you plan to remain their primary caregiver.
The cats applied for are over 12 weeks old,

Application Procedure

The Application is here. It's provided in pdf-format, which can be viewed with most modern browsers or with Adobe reader. If you can't display the form, you can download the free Adobe Reader from here.

Fill it out online (Tab key steps through form; Shift-Tab steps backward; use Spacebar fill in appropriate check boxes) -- or print out the form and fill out by hand using black ink and printing legibly. Sign and date it and return it to us at the address above and include the following supporting information:

Proof of income. Copy of the first page of your most recent federal income tax return or if you are on food stamps. a copy of your Eligibility Notice.
Proof of address. Copy of utility bill, phone bill or lease verifying your zipcode is within our service area.

Re-application Procedure

If you have had less than 3 cats previously fixed through any of our spay/neuter programs and had abided by the program requirements, you may use this application to fix additional cats provided you stay under the 3-cat limit. Cats an be either indoor- or outdoor-living cats provided you are committed to their life care.

Application Timetable

Once you mail your complete package to us, give us 3-5 working days to process your request. If you're approved, your vouchers will be mailed to you at that time. If you don't receive a mailing from us within 10 days of when you send us your application, call us at 734-663-8000 to find out what happened.
